Practicum Advisor Form: You will need to find a facility to complete your pracitcum assignments and be able to check in with a Practicum Advisor once these tasks have been completed. The Advisor has to meet one of the following requirements:
- an Activity Director Certified (ADC)
- an Activity Consultant Certified (ACC)
- a Certified Therapeutic Recreation Specialist (CTRS)
- a Department Head in your facility with at least 5 years of experience
- or the Administrator
This person must sign off on all of your practicum assignments and must be able to meet with you at least once a week. Please submit their information by completing and faxing the "Practicum Advisor Form" provided to the number listed on the form.
Midterm Practicum Pack: Print this packet, keep it in a binder, and record your hours until it is completely filled out. The forms will list tasks to be done by you at your facility where you work or where you volunteer. Once the entire packet is completed, upload or fax in your midterm Sign-off Forms (end of 8th week).
Final Practicum Pack: Print this packet, keep it in a binder, and record your hours starting Week 9 until it is completely filled out. The forms will list tasks to be done by you at your facility where you work or where you volunteer. Once the entire packet is completed, fax in your midterm Sign-off Forms(end of 16th week).
Sign-Off Forms: Log your hours, the form lists the task, the date when you started and when you finished, indicate your hours, have your practicum advisor initial and you initial.
We offer the Modular Education Program for Activity Professionals Part I & II, also known as the MEPAP. Both courses follow the National Certification Council for Activity Professionals curriculum and guidelines, and are applicable toward national certification through NCCAP. The MEPAP class is defined as a qualifying credential to be an activity director in many state regulations.

All MEPAP classes (both classroom and independent study) require a 90 hour practicum, to be completed in conjunction with the course. The practicum assignments are a combination of written assignments, practical application of group and 1-1 techniques, and implementation of professional tasks. Practicum assignments are completed at an approved site and under the guidance of a practicum supervisor. All practicum assignments are designed to be useful and helpful in the participants future as an activity leader and manager.
